The following are violations observed: #204, <br />305,310 <br />204. (Repeat) The designated operator failed to document all the alarms from the attached alarm history <br />on the March 2013 designated operator monthly inspection report and failed to check that they were <br />responded to appropriately. The missing alarms include: Fuel Alarm for Diesel Fill on February 22, <br />2013. During the monthly inspection, the designated operator shall review the alarm history for the <br />previous month, check that each alarm was documented and responded to appropriately, and attach a <br />copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. Submit document of the response to the fuel alarm to EHD by <br />February 14, 2014. <br />305. The 91 -octane STP sump sensor failed to activate an audible and visual alarm when tested to <br />ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to <br />activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. <br />The sensor was replaced and retested during the inspection. A "UST Retrofit Verification with Inspector <br />Already Onsite" form has been completed and provided to the operator and contractor, <br />310. (Repeat) Approximately a liter of liquid was observed in the Diesel fill sump. If water could enter <br />into the secondary containment by precipitation or infiltration, it must be removed and disposed of <br />properly. Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous <br />waste regulations, and manage it accordingly. Ensure that the sump is maintained free of liquid. <br />Technician has removed the liquid and placed it in the hazardous waste container on site. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Title <br />EHD P,42 l9611 9(/1911f / C/ Page 3 UST INSPECTION REPORT <br />
