Financial responsibility documents were last completed on June 27, 2011, and the current financial <br /> responsibility document have not been submitted to the EHD. Current financial responsibility documents <br /> are required to be submitted annually by the time of the Routine inspection. Complete and submit a <br /> copy of the financial responsibility by July 22, 2012. <br /> 109. Annual monitoring system certification including annual line testing, leak detector testing, and spill <br /> container testing were performed on June 27, 2011, and the test report was received by the EHD on <br /> August 16, 2011. A copy of the test report must be submitted within 30 days of the tests. Immediately <br /> begin submitting a copy of the test report to the EHD within 30 days of the tests. <br /> 204. The designated operator failed to document all the alarms and attached alarm history on the <br /> February 29, 2012 and June 29, 2011 designated operator monthly inspection reports. The missing <br /> alarms include: February 1, 2012 and February 16, 2012, and at least June 27, 2011. During the <br /> monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history <br /> with documentation taken in response to any alarms to the monthly report. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. Observed Doug (designated operator) documented on site the alarms on the February <br /> 2012 designated operator inspection report, and attached a copy of"equivalent' alarm history. Violation <br /> for February 2012 designated operator inspection report was corrected on site.
