PUBLt HEALTH SERICES
SAN JOAQUIN COUNTY r` i
JOGI KHANNA NI.D.,NIT H. 3
Health Officer
qui......
P.O. Box 2009 (1601 East Hazelton Avenue) • Stockton, California 95201 FOR
(209) 468-3400
GEOR E AM OAK UCI COPY 'JAN 0 3 lyy%
LODI CA 95240
RE: George Yamauci
5413 Live Oak Rd.
Lodi, CA 95240
San Joaquin County Public Health Services,Environmental Health Division(PHS/EHD)has recently reviewed
the analytical results of the soil and/or water results taken from beneath the underground storage tank(s) that
was/were removed from this site in November 1986.
Current standards, laboratory detection limits and/or your site specific characteristics (i.e. depth to seasonal
high groundwater, nearby wells, septic systems) make it impossible for PHS/EHD to evaluate your site for
closure.
An "Underground Storage Tank Unauthorized Release(Leak)/Contaminated Site Report"has not been filed
for this site nor is it considered contaminated. However, if you find site closure certification is necessary,
PHS/EHD would not be able to provide it without further site investigation.
Should you wish to further investigate your facility, PHS/EHD has been designated by the State of California
Water Resources Control Board as the local agency overseeing the investigation of leaking UST"s. Contact
the PHS/EHD Site Mitigation Unit for further information regarding this matter at (209) 468-3450.
Jogi Khanna, M.D., M.P.H.
Health Officer
Donna Heran, REHS, Program Manager
Environmental Health Division
DH/MM:lb
A Division of San Joaquin Courcy Health Care services
