State law provides <br /> for a fee system to cover t•.he cost of implementing this skate rnandat:ed <br /> p rog ratn. <br /> Local fees (see .attached Fee Schedule) , for these required inspectional <br /> services, will be billed on a Yearly basis. The facilities Permit to Operate <br /> will. be :issued for a five Year period, In addition to the yearly <br /> inspectional fee, this statement will. -..15o include a $56/tank State surcharge <br /> fee. The State surcharge fee per tank will be chArged every five years or <br /> whenever the facilities permit is renewed or, amended. 011 State surcharge <br /> fees will be transmitted by the Health District 1:o the California Water <br /> Resources Control Board. <br /> To apply -for, a faci..lities Permit to Oper., c":?mplete the aktat::hed Fee <br /> Worksheet and submit the appropriate feet::.
