HWY 99 EAST. <br />The above referenced application has been APPROVED WITH <br />CONDITIONS. The following are the conditions of approval for <br />this permit: <br />1. The contractor shall be properly registered with Public <br />Health Services -Environmental Health Division (PHS-EHD). The <br />proposed contractors have been reported as: Paradiso, Fillner, or <br />Gettler-Ryan. <br />2. The continuous monitoring system shall: <br />a. be connected to an audible and visual alarm system and <br />the pumping system. <br />b. shut down the pump and activates the alarm system when a <br />released is detected. <br />C. shut down the pumping system automatically if the <br />continuous monitoring system fails or disconnected. <br />OR <br />An annual line test shall be conducted on all underground <br />pressurized piping with secondary containment. <br />3. Installation of a Veeder-Root 8473 Magnetostrictive Probe as <br />indicated in the submitted application. <br />4. Evidence of financial responsibility must be on file with <br />PHS-EHD in order for the underground storage tank systems located <br />at this site to be in compliance. <br />5.
