ELEYi
7-Eleven, Inc.
Stephen Boyd
Direct Dial Number(714)771-5484
Cellular(916)340-4182
Via Email Delivery
May 22, 2015
San Joaquin County Environmental Health Department
600 East Main Street
Stockton, CA 95202-3029
Attn: Elena Manzo /
Re: 7-Eleven Store No.: 32190 R
4943 South Highway 99 a CLr//EL
Stockton, CA
Inspection Report e MAY 2 20+5
Dear Ms. Manzo: ' iNV/Ro
NF4'ru n C I
The intent of this letter is to respond to the items you noted on your Inspection Reric `3,/,fp'ce
location referenced above dated 5/1/2015 which were not close at the time of your inspection.
Item 1: During your inspection you observed that L7 and L12 alarms had occurred on 3/19/2015 and you
were unable to locate documentation of those alarms. You requested documentation to be sent to you.
Resolution 1: During our conversation on the day of your inspection, I had identified that these alarms
had occurred during, or as part of, a vapor recovery test which took place on 3/19/2015. A copy of that
test report was sent to you for your records. Additionally, due to this occurrence, I have instructed our
tester to print an alarm history upon completion of any work they perform at our locations which can
affect, but is not directly related to, the monitoring system. This will ensure any alarms which may have
occurred will be identified and documented.
Item 2: On your inspection report you noted that the hire dates for several employees were not
documented on the Designated Operator (DO) training form and asked that an updated training form be
submitted.
Resolution 2: It should be noted that not all store employees are forthcoming with their hire dates. I was
able to obtain the hire dates for the employees who are still currently employed at this location and have
had the DO update the training records. A copy is attached for your records.
This letter should address the item(s) noted on the Inspection Report. 7-Eleven assumes that all issues
regarding this report have been resolved and are closed. The submittal of this response by 7-Eleven, Inc.
shall not constitute, nor be deemed to constitute, an admission of liability or responsibility with respect to
the alleged violations and, in addition, evidence of this settlement may not be used in any administrative or
judicial proceeding or otherwise.
'7-tilcven.bic.
P.O.Box 711—Dallas.7exiv 75'21-0711
