PERMIT#: 2341 <br /> 4005 Port Chicago Hwy. #OF TANKS:2 <br /> Concord, CA 94520-1120 SITE NAME: Pacific Bell <br /> SITE ADDRESS: 1060 Terven. <br /> Salinas,CA 93901 <br /> This permit is granted to the above named business for temporary closure of underground storage tanks in which storage <br /> has ceased but where the owner or operator proposes to close or upgrade or use the underground storage tank within one <br /> Tear. <br /> The owner or operator shall comply will all the following requirements to complete and maintain temporary closure of an <br /> underground storage tank: <br /> �1. All residual liquid,solids,or sludge's shall be removed and handled in accordance with the applicable provisions of <br /> Chapters 6.5 and 6.7 of Division 20 of the health and Safety Code. <br /> Except for required venting, all fill and access locations and piping shall be sealed using locking caps or concrete <br /> plugs. <br /> 3. Power service shall be disconnected from all pumps associated with the use of the underground storage tank unless <br /> the power services some other equipment which is not being closed, such as the impressed-current cathodic <br /> protection system. <br /> The monitoring required pursuant to this permit may be modified by this department during the temporary closure <br /> period. In making a decision to modify monitoring requirements,this department shall consider the need to maintain <br /> monitoring in order to detect unauthorized releases that may have occurred during the time the underground storage <br /> tank was used but that have not yet been detected. In all cases,corrasion protection shall continue to be operated. <br /> The owner or operator shall inspect the underground storage tank at least once every three months to verify that the <br /> temporary measures are still in place. The inspection shall include but is not limited to the following: <br /> A. Visual inspection of all locked caps and concrete plugs. <br /> B. If locked caps are used, at least one shall be removed to determine if any liquids or other substances have <br /> been added to the underground storage tank. <br /> At the end of a temporary closure period over 12 months, including any extension granted by this department, the <br /> owner may reuse the underground storage tank(s) only if the tank(s) meet the requirements of Article 3 for new <br /> underground storage tanks or is upgraded to meet the requirements of Article 6. <br /> All new and existing underground storage tank systems which have been temporarily closed must continue to comply <br /> with repair and record keeping requirements, release reporting and investigation requirements,and release response <br /> and corrective action requirements as specified in Title 23, Division 3 and Chapter 16 of the California Code of <br /> Regulations and Chapter 6.7 of the Health and Safety Code. <br /> The Monterey County Health Department Operating Permit conditions and fees shall be kept current during the <br /> closure period. <br /> ERMIT EXPIRES <br /> ix(6)months from Allen J.Strok R.E.H.S,M.P.A, <br /> Dir c a vironmental Health <br /> �eeember 16,2002 t <br /> (JoinRamirez,R.E.H.S. <br /> upervising Hazardous Materials Specialist <br /> tev.12/07101) <br />
