A copy of the fire department permit must be submitted at the time of removal. <br /> 2. Schedule removal appointment 48 hours in advance. <br /> 3. This tank is to be purged and rinsed prior to removal. <br /> 4. In addition, the following information must be submitted within 30 days of the removal <br /> date: <br /> (a) Hazardous Waste Manifest for piping, rinsate, residual fuel, or waste oil receipt if <br /> applicable <br /> 5. All fill and access locations and piping shall be sealed using locking caps or concrete plugs. <br /> 6. This permit expires on March 9, 2003. <br /> 7.
