• Date: 1/16/95 <br /> Emergency Response Pian (cont.) <br /> Facility Name: Pacific Bell, 4051 Newton Road, Stockton Facility ID: SKTNCAI6 <br /> In case of fires, the local Fire Department and Pacific Bell Emergency Control Center will be notified prompt- <br /> ly by the operator of the underground tank. <br /> In each step of notification, good documentation will be prepared to include: <br /> 1) Location <br /> 2) Specific description and location of the area involved <br /> 3) Time and date of occurrence <br /> 4) Description of spill and type of material <br /> 5) Approximate quantity of material that spilled/leaked <br /> All recordable leak and spill records will be maintained on the site. <br /> The key personnel on the site (Building Warden and Site Manager) are trained for responding appropriately to <br /> any major catastrophe. <br /> A training program conducted by the training department of Pacific Bell provides the information to the key <br /> personnel regarding emergency telephone numbers, reporting requirements, location of emergency equipment <br /> and evacuation procedures. <br /> This information is true and correct. If there is any change which would materially effect the Hazardous <br /> Material Management Plan, we will amend the appropriate sections. <br /> pg ' 8 <br />
