BOX 191 1205 E. NORTH STREET <br /> MANTECA, CA 95336 MANTECA, CA 95336 <br /> In late 1963Y the Governor signed into law A02013 and 1362. These Bills require <br /> the inventory, inspection and permitting of all underground storage tanks that <br /> contain hazardous materials. The San Joaquin Local Health District, Division <br /> of Environmental Health was designated as the enforcement agency for the <br /> cities and unincorporated areas within San Joaquin County. State law provides <br /> for a fee system to cover the cost of implementing this state mandated <br /> program. <br /> Local fees (see attached Fee Schedule) , for these required inspectional <br /> services, will be billed on a yearly basis. The -to Operate <br /> will be issued for a five year period. In adfitibn to the yearly <br /> A kude a $S6/tank State surcharge <br /> inspectional fee, this statement will ilso in(;� <br /> fee. The State surcharge fee per ta 4 1110 N�charged every five years or- <br /> e <br /> whenever the facilities-00it is relve"d or amended. All State surcharge <br /> fees will be transmitted by the Health District to the California Water <br /> Resources Control Board. <br /> To apply for a facilities Permit to Operate, complete the attached Fee <br /> Worksheet and submit the appropriate fees. Fees are due and payable February <br /> 3, 1986. Fees received after March 6, 1986 are subject to 100% penalty. <br /> In order to answer any questions regarding this Underground Storage Tank <br /> program, the San Joaquin Local Health Distict Division of Environmental Health, <br /> has scheduled the following information workshops: <br /> I* Thursdayy January 30, 1984, 10 AM <br /> 2" Monday, February 3t 1986, 2 PM <br /> 3. Tuesday, February 11, 1986, 2 PM <br /> All of the above informational-workshops will be conducted at the San Joaquin <br /> Local Health District Board Room, 1601 E. Hazelton Ave. , Stocktonp CA. Due to <br /> limited space, these worishops wil be limited to the first 200 persons per <br /> meeting. <br /> if you have any questions regarding this, please contact C.
