A plot plan showing the tank location is attached. There are <br /> no septic tanks on the property and no known wells within 500 <br /> feet. <br /> 3. A completed copy of the contractors questionnaire is attached <br /> as well as certificate of workers' compensation insurance. As <br /> the policy expires 01-01-88, a renued policy will be forwarded <br /> to you directly. <br /> 4. The tank is a 300 gallon tank that contained regular gas. <br /> 5. Analysis of soils will be performed by American Environmental <br /> Management Corporation's Lab (State Certification No. 210) . A <br /> "Release of Information Authorization" form signed by the facility <br /> owner is enclosed. <br /> 6. Any residual liquids, solids, or sludges will be landfilled. <br /> 7. To preclude explosion, dry ice will be inserted through fill lines <br /> into the tank to purge the tank of flammable vapors. AEMC's procedures <br /> require a drop to a reading of 10% of L.E.L. before removing the tank. <br /> 8.
