In each step of notification , good documentation will be prepared <br /> to include : <br /> 1 ) Location <br /> 2) Specific description and location of the area involved <br /> 3 ) Time and date of. occurence <br /> 4 ) Description of spill and type of material <br /> 5 ) Approximate quantity of material that spilled/leaked <br /> All recordable leak and spill records will be maintained on the <br /> site . <br /> The key personnel on the site (building warden and all the <br /> supervisors) are trained for responding appropriately to any major <br /> catastrophe . <br /> A training program conducted by the local safety department of <br /> Pacific Hell protides the information to the key personnel <br /> regarding emergency telephone numbers , reporting requirements , <br /> location of emergency equipment , and evacuation procedures. <br /> This information is true and correct . if there is any change which <br /> would materially affect the Hazardous Materials Management Plan , we will <br /> apply for an appropriate amendment .. <br /> Concur : <br /> A .
