o Plan for monitoring of potential exposure hazards to <br /> workers and the community. <br /> o Personal protective equipment to be used for each job <br /> classification. <br /> 0 site map indicating work, contamination reduction, and <br /> support zones, including security measures. <br /> 0 Procedures to be used for decontamination of personnel, <br /> personal protective equipment, and excavation equipment. <br /> 0 Safe work practices. <br /> o Medical and emergency services to be used, including <br /> route to the nearest hospital. <br /> o Any standard operating procedures (e.g. , respirator fit <br /> test procedures) . <br /> o Personnel training programs required. <br /> o Company medical surveillance program. <br /> o Format for record keeping. <br /> 9. 0 POST-CLOSURE MAINTENANCE <br /> It is expected that no hazardous waste will remain once the tanks, <br /> piping, and soils are removed. If contamination is indicated from <br /> the soil or water analyses, or from visual evidence during tank <br /> removal operations, further investigations will be implemented. <br /> 10. 0 AMENDMENTS <br /> This closure plan will be amended when a change in procedure or <br /> schedule is necessary. Amendments will record the change and the <br /> date it is effective. PHS/EHD will be notified of the amendment 30 <br /> days prior to the effective date. The closure plan will be kept on <br /> file at the job site. <br /> 11. 0 NOTIFICATIONS <br /> A written notification will be given to the PHS/EHD 20 days prior <br /> to the expected date of closure. Permits and written notification <br /> are also required for submittal to the PHS/EHD, the local fire <br /> department and possibly. for the San Joaquin County Air Pollution <br /> fControl District as well. <br /> 12 . 0 CERTIFICATION <br /> Upon completion of closure, the contractor will submit a certifica- <br /> tion, validated by a registered professional engineer, that the <br /> facility has been closed in accordance with the approved closure <br /> plan and all applicable California state laws. <br />
