NOTE: All farm underground storage tanks which contain <br />other than motor vehicle fuels, such as waste oil <br />or other chemicals, are currently subject to all <br />requirements of the California Underground Tani; <br />Regulations. <br />Farm tank owners who have already closed farm <br />tanks by removal or closure in place prior to July <br />1, 1969, complete the enclosed UNDERGROUND <br />STORAGE TANK PROGRAM -FARM TANK INFORMATION FORM <br />and return to the San Joaquin Local Health <br />District - Environmental Health Division with your <br />farm UGST billing statement. <br />Any soil and/or groundwater contamination <br />identified during operation or closure of any <br />underground tank, whether exempt or regulated, <br />must be reported to the San Joaquin Local Health <br />District and the California Regional Water Quality <br />Control Board (CRWOCB). <br />Owners of farm tanks not previously registered <br />with the San Joaquin Local Health District or the <br />State Water Resources Control Board(SWRCB), <br />complete the enclosed UNDERGROUND STORAGE TANK <br />PROGRAM -FARM TANK INFORMATION FORM and return it <br />to the Environmental Health Division. <br />If you have any questions regarding the Farm UGST Program or the <br />enclosed billing statement, please contact the San Joaquin Local Health <br />District - Environmental Health Division at (209) 466-3422 between 6:00 <br />am and 5:00 pm, Monday - Friday. <br />Jogi Khanna, M.D., M.P.H. <br />Distrito Health Officer <br />Roti Yat n ti, irl a <br />Environmental Health Division <br />RV:asm <br />
