L11 sensor on the auto side went <br /> into alarm on 8122/2015, 919/2015 and 91102015, as noted by the designated operator (DO); however, the alarms were <br /> not addressed by the DO and maintenance records associated with these alarms were not found on site. L22 sensor, <br /> on the truck side, went into alarm on 1127/2016 and went into alarm three times on 512012016 but the alarms were not <br /> documented by the DO nor were maintenance records associated with these alarms found on site. These records <br /> shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or <br /> sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) <br /> results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring <br /> equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. <br /> Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit <br /> copies to the EHD. <br /> This is a minor violation. <br /> 203 CCR 2712(b) Failed to maintain records of monitoring, repair, lining, and upgrades on site. <br /> Monitoring system certification testing reports were not found on site during the inspection. Also records of repairs <br /> associated with the 1-11 sensor alarms on 8122/2015, 91912015, 9/10/2015 and L22 sensor alarms on 1127/2016 and <br /> 5/20/2016 for the UST system were not found on site. These records shall be kept on site for the life of the tank. <br /> Immediately locate and maintain these records on site and submit copies to the EHD. <br /> This is a minor violation. <br /> 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans was not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class II. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the January 29, 2016 <br /> and May 28, 2016 designated operator monthly inspection reports and failed to check that they were responded to <br /> appropriately. The missing alarms include: 1-22, brine sensor in dispenser 17A/18B on the truck side, on January 27, <br /> 2016 and three times on May 20, 2016. During the monthly inspection, the designated operator shall review the alarm <br /> history for the previous month, check that each alarm was documented and responded to appropriately, and attach a <br /> copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required information on the <br /> reports. <br /> This is a repeat violation, Class II. <br /> Page 5 of 9 <br />
