• • <br /> The following is an itemized list of underground storage tank violations that have not <br /> been addressed for Fast Lane Central Valley as of October 16, 2017. <br /> Open violations from June 20,2017 inspection <br /> Violation#101 -No permit to operate. <br /> A permit to operate the UST system was not found at the site. No person may own or operate an UST unless a <br /> permit for its operation has been issued by the local agency to the owner or operator of the UST system. <br /> Immediately obtain a permit to operate a UST system from the EHD. An"Operating without a Permit"penalty <br /> applies. <br /> Violation#103-Current financial responsibility documents not submitted. <br /> Financial responsibility documents submitted to California Environmental Reporting System(CERS)are not <br /> accepted by the Environmental Health Department(EHD). Under Section A of Certification of Financial <br /> Responsibility, change 500,000 dollars per occurrence to 1 million dollars per occurrence,add"per occurrence and <br /> annual"after$995,000; and add"per occurrence and annual aggregate"after$5,000. The Letter from Chief <br /> Financial Officer lists$5,000,000 for#4(Tangible net worth(subtract line 3 from line 2).The correct amount should <br /> be$500,000. Also the Letter from Chief Financial Office states that"property damage caused by an unauthorized <br /> release of petroleum in the amount of at least$ 1 Mil annual aggregate coverage.The$ 1 Mil should be changed to <br /> $5,000 to agree with number 1.Amount of annual aggregate coverage being assured by this letter....$5,000. <br /> Current financial responsibility documents are required to be submitted annually. Immediately log into the CERS)at <br /> http://cers.calepa.ca.gov/, and upload the revised documents. <br /> Violation#104-UST operating permit application for facility and tank information not submitted or current. <br /> UST Facility Operating Permit is not current in CERS.The Board of Equalization(BOE)number is not valid.Any <br /> change of information must be updated in CERS within 30 days of the changes. Immediately contact the BOE and <br /> get a valid BOE number. Log into CERS, update the required information, and submit for review by the EHD. <br /> UST Tank Information forms for the all five underground storage tanks are not current in CERS. Under Tank <br /> Construction:change"Yes"to"No"for Fill Tube Shut-off Valve; <br /> Under Corrosion Protection, change"No"to"Yes"for Isolation. <br /> For Tank ID#1,2,4 and 5-under Vent,Vapor Recovery(VR)and Riser/Fill Pipe Piping Construction, change <br /> "Fiberglass"to"Steel"for Riser Pipe Primary Containment. <br /> For Tank ID#3,4 and 5-under Vent,Vapor Recovery(VR)and Riser/Fill Pipe Piping Construction, remove <br /> "Fiberglass"from Vapor Recovery Primary Containment and remove"Fiberglass"from Vapor Recovery Secondary <br /> Containment. <br /> For Tank ID#4 and 5-under Tank Description,remove"One in a Compartmental Unit"and enter"A Stand-alone <br /> Tank"for Tank Configuration. <br /> Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, <br /> update the required information, and submit for review by the EHD. <br /> Violation#105-Failed to have an approved UST monitoring plan. <br /> The monitoring plan is not current and/or not approved by the EHD. <br /> For all five underground storage tanks: under Pipe Monitoring is Performed Using the Following Methods, add the <br /> brine interstitial 304 sensor to"Leak Sensor Model", and add"Yes"to Leak Alarm Triggers Automatic Pump <br /> Shutdown, and"Yes"to Failure Disconnect Triggers Pump Shutdown; beneath Under Dispenser Containment <br /> (UDC)Monitoring,add the brine interstitial 304 sensor, under Recordkeeping,change"No"to"Yes"for Equipment <br /> Maintenance and Calibration Records. <br /> The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log <br /> into CERS,make the necessary changes, and submit for review by the EHD. <br /> Violation#107-Plot plan/site map not submitted or failed to completely show where monitoring is <br /> performed. <br /> An accurate UST Monitoring Site Plan was not submitted. A site plan is a facility map that must be submitted <br /> identifying the locations where monitoring will be performed. Immediately log into the California Environmental <br /> Reporting System(CERS)at http://cers.calepa.ra.gov/, remove the Underground Storage Tank Monitoring Plan <br /> currently in CERS, and upload a copy of the UST Monitoring Site Plan(map). <br /> Page 1 of 9 <br />
