,%we %.W <br /> June 20, 1995** I received and reviewed the laboratory soil sample results. <br /> The lead level (Pb = 190 ppm) in the stockpile was higher than the allowable <br /> limit of 10 times the Soluble Threshold Limit Concentration (STLC) of 5 ppm. <br /> I phoned Chris Fisch and requested STLC (wet test) for lead. (6-9-95 STLC <br /> Results 2 F) <br /> July 17, 1995 <br /> PHS-EHD received a copy of Workers Compensation Insurance from Sigerseth <br /> Insurance Associates for Fisch Environmental Construction Services. <br /> (insurance 3 0) <br /> July 20, 1995** Site follow up revealed wet test results have not been <br /> received. <br /> July 31, 1995** I received a letter (work plan for re-excavation) from David <br /> Fisch, over the counter, covering the re-sampling of the UST excavation. Mr. <br /> Fisch stated STLC test results from 6-9-1995 removal have been sent to PHS- <br /> EHD. (Work Plan 2 H) <br /> September 5, 1995** I received a phone call from Chris Fisch to set up the <br /> re-sampling inspection for September 7, 1995. <br /> September 7, 1995** I witnessed the re-excavation of the former UST location <br /> by Fisch Environmental Contractors employee, Rick Bertolini, and soil sampling <br /> of the excavations bottom by Geo Analytical Laboratories. The stockpile soil <br /> is to be transported to Kettleman Hills by Jack Le Strange, Le Strange <br /> Trucking, CA uniform manifest #90026530. (Manifest 2 K) <br /> September 8, 1995** I requested and received a copy laboratory analytical <br /> results of STLC for stockpile lead sample from 6-9-1995, from GeoAnalytical <br /> Laboratories. (6-9-95 STLC Lab Results 2 F) <br /> September 12, 1995** I phoned Fisch Environmental and spoke to Chris Fisch. <br /> PHS-EHD has not received a copy of UST tracking record or the rinsate manifest <br /> from 6-9-95 UST removal. Chris told me she would have David Fisch call me as <br /> soon as he returned. <br /> September 25, 1995** Received and reviewed laboratory soil sample results <br /> from the tank excavation bottom at 7 ft & 5 in taken during re-sampling <br /> inspection 9-7-95. <br /> Phoned Chris Fisch and requested complete information for tank closure. Chris <br /> told me she had already mailed the closure packet. <br /> September 26, 1995** I received and filed Fisch Environmental closure Packet <br /> in the appropriate attachment sections. <br /> July 9, 1996** Kasey Foley requested I update this D.A. case request for <br /> David Irey. <br /> July 10, 1996** I called Ron Hiltscher, and requested information on the <br /> Uniform Fire Code section which would apply to UST cutting/hot work violation <br /> for work performed by Fisch Environmental Construction without COS Fire <br /> Prevention approval or permit. Mr. Hiltscher told me tank cutting was <br /> recognized as not safe by just about any recognized (fire) standard. He <br /> referred me to section 101.4 of the 1994 - 1995 Fire Code supplemental rules <br /> and regulations. <br /> August 22, 1996 <br /> Reviewed UST Owners and Contractor files, determined format, and attachment <br /> order. <br /> 5 <br />
