Provide parcel plot plan showing tank(s) to be removed and <br /> tank(s) distance from all premises, septic tanks, buildings, <br /> property lines, and all wells within 500 feet. <br /> Figures 1 is a map of the site showing the locations of the <br /> tank sites and buildings. There are no septic tanks, wells or <br /> residences within 500 feet of the tank-removal site. <br /> Figure 2 is a map showing the approximate locations of where the <br /> tanks were removed in relation to nearby structures. <br /> 3. Provide name of contractor, including contractor's <br /> classification, license number, certificate of workers <br /> compensation insurance. <br /> The tanks have been removed by Simpson Paper Company. <br /> 4. Identify tank(s) size and product(s) currently and/or <br /> previously stored in each tank. <br /> UST # 1 : 300 gallons, regular gasoline <br /> UST # 2: 400 gallons, regular gasoline <br /> 5. Provide the name of the laboratory that will collect soil <br /> samples and a "Release of Information Authorization" form signed <br /> by the facility owner/operator. <br /> Samples will be collected from native soil immediately below each <br /> tank site (now filled and covered) using a hollow-stem auger. A <br /> sample will be obtained through the center of the auger using <br /> a split-spoon sampler. Figure 2 shows the locations of the <br /> borings. <br /> The samples will be analyzed by the following laboratory: <br /> Curtis & Tompkins <br /> 2323 5th Street <br /> Berkely, CA 94710 <br /> Analyses will include: BTX, TPHC, Lead, EDB, and tetraethyl <br /> lead. <br /> -2- <br />
