1 IX. HAZARD MITIGATION <br />Identify procedures to mitigate all hazards listed in Section VI bYP g lacin the task number next to the <br />appropriate mitigating measure. Listing of standard procedures is not inclusive. A specific procedure <br />' must be entered to mitigate each hazard identified in Section VI. <br />Activity <br />List Number A. Mechanical Hazards <br />1 Do not stand near backhoe buckets and earthmoving equipment. <br />1 Verify that all equipment is in good condition. <br />1 Do not stand or walk under elevated loads or ladders. <br />1 Do not stand near unguarded excavation and trenches. <br />1 Do not enter excavation or trenches over 5 feet deep that are not <br />properly guarded, shored, or sloped. <br />1 Consult DHSO if other mechanical hazards exist. <br />B. Electrical Hazards <br />1 C. Notification within 48 hours prior to <br />scheduled drilling or excavation work. <br />1 <br />In addition utilities to cleared by a private utility locator on the day of <br />drilling or excavation work. <br />1 <br />Maintain at least 10 -foot clearance from overhead power lines. <br />1 <br />Contact utility company for minimum clearance from high voltage <br />power lines. <br />1 <br />If unavoidably close to buried or overhead power lines, have power <br />turned off, with circuit breaker locked and tagged. <br />1 _ <br />Properly ground all electrical equipment. <br />' <br />1 <br />Avoid standing in water when operating electrical equipment. <br />1 <br />If equipment must be connected by splicing wires, make sure all <br />connections are properly taped. <br />1 <br />Be familiar with specific operating instructions for each piece of <br />' <br />equipment. <br />1 C.
