St. Joseph's Medical Center <br /> 2013 CUPA Inspection Return to Compliance <br /> Hazardous Waste Program Inspection Report Nov. 5, 2013 <br /> Violation:Item#605 CCR 66262.34(f) <br /> Failed to completely label containers or tanks of hazardous waste. <br /> a. Pharmacy:One container was labeled incorrectly. <br /> b. Pharmacy:One container was labeled incorrectly. <br /> c. Pathology:One box was labeled incorrectly,and needs to be disposed of. <br /> Correction: Items A and B were corrected onsite during the inspection. Item C was immediately labeled <br /> And the box was relocated to our hazardous waste storage area_It is scheduled for pickup by our <br /> hazardous waste hauler by December 7, 2013. <br /> Violation:Item#804 CCR 66273.33(b) <br /> Failed to store UW lamps in a closed container and in a manner that prevents breakage or release to the <br /> environment. <br /> Correction: Lids were put on. On November 13 and 19 2013,it was discussed in our weekly crew <br /> meeting that the light tube containers must be kept with the lids on.We will continue to discuss this at <br /> our crew meetings until it becomes standard operating procedure. <br /> Violation: Item#808a CCR 66273.34 <br /> Failed to label universal waste to identify the type of universal waste_Containers of fluorescent tubes <br /> were not labeled as universal waste. Failed to label universal waste <br /> Correction: Corrected during the inspection,staff has been notified that as new barrels are dropped off <br /> they are to be labeled properly. <br /> Violation: Item#808b CCR 66273.34 <br /> Failed to label universal waste electronic devices.One box of waste ballasts were not labeled as <br /> required. <br /> Correction:The container was switched out to a 5 gallon plastic bucket and labeled "Universal Waste— <br /> Electronic Devices".The staff was educated on the proper labeling for this type of waste. <br /> Violation: Item#808c CCR 66273.34 <br /> Failed to label universal waste batteries.Two containers of batteries in the cafeteria area were not <br /> labeled as required by this section. <br /> Correction:Labels stating"Universal Waste—Batteries"have been ordered for every battery waste <br /> container throughout the medical center. <br /> SII <br /> f <br />
