Pharmacy:One container was labeled incorrectly. <br /> b. Pharmacy:One container was labeled incorrectly. <br /> c. Pathology:One box was labeled incorrectly,and needs to be disposed of. <br /> Correction: Items A and B were corrected onsite during the inspection. Item C was immediately labeled <br /> And the box was relocated to our hazardous waste storage area_It is scheduled for pickup by our <br /> hazardous waste hauler by December 7, 2013. <br /> Violation:Item#804 CCR 66273.33(b) <br /> Failed to store UW lamps in a closed container and in a manner that prevents breakage or release to the <br /> environment. <br /> Correction: Lids were put on. On November 13 and 19 2013,it was discussed in our weekly crew <br /> meeting that the light tube containers must be kept with the lids on.We will continue to discuss this at <br /> our crew meetings until it becomes standard operating procedure. <br /> Violation: Item#808a CCR 66273.34 <br /> Failed to label universal waste to identify the type of universal waste_Containers of fluorescent tubes <br /> were not labeled as universal waste.
