CONSENT <br />Consent to conduct inspection that involves: taking photographs, reviewing and copying records, <br />questioning personnel and inspecting hazardous waste handling areas. <br />Consent given by (name and title): Chris E. Manifests, Bills of Lading, LDR's and Exception Reports: <br />Prior to conducting the inspection I reviewed our Manifest database. I did not observe any manifest <br />discrepancies from the database. In reviewing manifests at the facility I observed one manifest <br />(20532578) that did not have a copy of the signature of receipt from the treatment, storage, or disposal <br />facility (TSDF) within the required time frame [see attached Summary of Violation (SOV)l. On <br />December 29, 2000, Advanced Interconnect Technologies (AIT) submitted to DTSC a copy of the <br />TSDF's signature of receipt page for the manifest in question (TSDF received the waste on the same <br />day it was picked up from AIT).
