There must be at least one <br /> rdinator on site or on call to coordinate emergency response measures, and the following information <br /> pby a phone: the name and phone number of the emergency coordinator; location of fire extinguishers, <br /> spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has <br /> a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. A form <br /> is provided that can be used for this purpose. <br /> This is a Class II violation. <br /> 105 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> At the time of inspection, it could not be demonstrated (not necessarily documented) that employees who handle <br /> hazardous waste were properly trained. Several unknown wastes were found throughout the facility, including a 5 <br /> gallon bucket in the hazardous waste storage area. According to Ms. Hardy, the employees"put stuff' in the <br /> hazardous waste area without labeling or other identification. The generator must ensure that all employees who <br /> handle hazardous waste are thoroughly familiar with proper waste handling and emergency procedures. Immediately <br /> provide training to all employees who handle hazardous waste and submit a copy of the training records to the EHD. <br /> This is a Class II violation. <br /> 107 CCR 66262.23(a) Failed to complete, sign or date manifest; or obtain dated signature of transporter. <br /> Manifest number 012864730JJK(5/16/14)was observed on site with an incomplete waste description. In box 9b the <br /> waste was listed as"non-RCRA hazardous waste solid (white powder)"and was sent under waste code 181. The <br /> generator and waste sections of the uniform manifest shall be completed and signed and dated by the generator. The <br /> transporter shall also sign and date the manifest. Ensure that all manifests are properly completed for any transport of <br /> a hazardous waste for off-site transfer, treatment, storage or disposal. <br /> This is a Class II violation. <br /> 108 CCR 66262.23(a)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> Generator copies of manifest numbers 012864730JJK(5/16/14)was not sent to DTSC within 30 days of shipment of <br /> hazardous waste. The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when <br /> California is either the generator state or the destination or consignment state. The generator manifest copy shall be <br /> mailed to: <br /> DTSC Generator Manifests <br /> P.O.
