II. SCHEDULE OF CORRECTIONS: <br /> Violation #1: Effective immediately,drums containing hazardous waste shall <br /> be labeled with the following information: <br /> a) The accumulation start date. <br /> b) The words "Hazardous Waste" <br /> c) The composition and physical state of the waste. <br /> d) Statement or statements which will call attention to the <br /> particular hazardous properties of the waste. <br /> e) Your name and address. <br /> Violation #2: Effective immediately, all drums containing hazardous waste <br /> shall always be closed, except when it is necessary to add or <br /> remove waste. <br /> When all violations have been corrected, please send written certification to this office at <br /> the letterhead address. This certification shall contain the sample certification statement <br /> indicated on Attachment 1. <br /> PHD-EHD will conduct a reinspection of your property to verify compliance. The issuance <br /> of this Report of Violation(s) and Schedule of Correction does not preclude PHS-EHD <br /> from taking any administrative civil or criminal action as a result of the violations noted <br /> herein. <br /> If you have any questions or require additional information regarding this report, please <br /> contact L.T.
