A written training program and training records were not available on site. Facility personnel shall <br /> complete a training program in accordance with Title 22 regulations. This program shall be directed by a <br /> person trained in hazardous waste management procedures and shall include the following: <br /> - hazardous waste management procedures, including contingency plan implementation <br /> - emergency procedures, equipment, and systems <br /> - key parameters for automatic waste feed cut-off systems <br /> - communications or alarm systems <br /> - response to fire, explosion, or groundwater contamination incident <br /> - shut down of operations <br /> The training program shall be completed by each employee within six months of hire, prior to <br /> unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br /> closure of the facility and shall include: <br /> - name &job title of each employee <br /> - written job description for each job title listed, including requisite skill/education/qualifications/duties <br /> - description of training to be given to each position <br /> - records that training was completed for each employee <br /> Immediately prepare and implement a complete hazardous waste management training program for this <br /> facility in accordance with Title 22 regulations and ensure that all employees complete this training at <br /> least annually. Submit a copy of the training program and records the the EHD by April 16, 2011. <br /> 60, 61, & 62. Manifest number 007139921JJK (8/10/10) was found without a signed off copy from the <br /> destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the <br /> destination facility on site for three years and have them readily available for review. If the generator did <br /> not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility <br /> to which the generator's waste was submitted within 35 days of the date the waste was accepted by the <br /> initial transporter, the generator shall contact the transporter and/or the owner or operator of the <br /> designated facility to determine the status of the waste. If the generator did not receive a copy of the <br /> signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial <br /> transporter, the generator shall submit an Exception Report that includes a legible copy of the missing <br /> manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste <br /> and the results of those efforts.
