Failure to file the required storage statement or to pay the appropriate fee is <br /> subject by law to civil monetary penalties. <br /> Enclosure 1 is a summary of our current information regarding the fee payment history of your <br /> facility based on previous storage statements. For your convenience, you may make any <br /> necessary changes directly on the summary sheet and use it as your storage statement; however, <br /> be sure all of the following information is included: <br /> 1. Name and address of the tank facility. <br /> 2. A contact person for the tank facility. <br /> 3. The total storage capacity of the tank facility. <br /> 4. For each tank that exceeds 10,000 gallons in capacity and which holds a <br /> substance containing at least 5 percent of petroleum: <br /> a. Location (on the facility) <br /> b.
