Home Agency(and Unit):
6. Resources Assigned:
Name ICS Position Home Agency(and Unit)
7.Activity Log:
Date/Time Notable Activities
7-11-2014 EM Received a notification from Sheryl (209-468-XXXX), general hospital regarding a 5-7
2:20 am gallon diesel spill at this 1000 E Roth Rd (UPRR). Called Brad 888-877-7267 (UPRR), was
- referred to Tomas(UPRR) who is working on it. Tomas reports 5-7 gallons spilling on the
ground; a falling container punctured the tank causing the release. Absorbent was applied,
spill stopped and area was secured. Tomas doesn't know if any further clean-up will be
conducted right away or in the morning but will find out and call me back. UPRR haz. mat
is working on it, no water was impacted and he is not sure if any excavation will be
necessary. Provided my contact information. At 2:3 received a phone call from Tomas
who is reporting that NRC was retained to conduct the clean-up and they will be onsite in 2
to 3 hours. He will call me if any changes are anticipated in the estimated arrival time.
Called Tomas at 4:20 am who confirmed that the estimated arrival is still accurate. Called
Kasey Foley (program coordinator) who recommended to visit the facility later but before 8
am. Called Tomas back and let him know that I will be stopping by the facility on my way to
work in the morning to verify clean-up. Arrived onsite at 7:25, called the dispatch
888-877-7267, was directed to an onsite person Dave and then to Jeremiah Farree,
209-546-7288. Jeremiah showed the two areas where the leakage occurred, one where
the semi was parked and one in front of the checkpoint location. Both areas had absorbent
applied to them. The swept up absorbent was placed in the labeled onsite hazardous
waste drum and will be disposed within the appropriate time frame (per hazardous waste
generator requirements). Downloaded and printed pictures, prepared Prop 65, complaint
and ER record, printed a map.
t
8.Prepared by: Name: Position/Title: Signature:
ICS 214, Page 1 Date/Time:
