Turner Road <br />Lodi, CA 95240 <br />RE: Response to Hazardous Waste Program Inspection conducted on December 2, 2011 <br />Dear Mr. Cacapit, <br />On December 2, 2011 the San Joaquin Environmental Health Department (SJEHD) conducted a <br />routine Hazardous Waste Program Inspection of Central Valley Waste Services (CVWS) located <br />at 1333 E. Turner Road, Lodi, CA. Central Valley Waste Services is a subsidiary of USA Waste <br />of California, Inc. During the inspection SCEMD documented nine alleged violations. CVWS <br />has subsequently investigated each alleged violation and has prepared the following response for <br />each. <br />Violation code: 122 — Failed to keep signed copy of manifest from designated facility for <br />three years. <br />It was noted in the inspection form that manifest numbers 001717155JJK, 002260997FLE, <br />002709571FLE,002709588 FLE, 002709695FLE, 002709733FLE, 002709961FLE, <br />003025078FLE, 003025226FLE, 003025415FLE, 003025496FLE, 003447059FLE, <br />003447220FLE, 003447421FLE, and 003931454FLE were not found on site. Manifest numbers <br />002260880FLE, 003447440FLE, 003930600FLE, 003930656FLE, 003930714FLE, <br />003930948FLE, 003931125FLE, 003931127FLE, 003931373FLE, 003931267FLE, and <br />003931374FLE were found without a singed off copy from the destination facility. Copies of all <br />signed manifests from the destination facility have been located and will be kept on-site for a <br />minimum of three years. A copy of each manifest is located in Attachment 1. <br />Violation code: 123 — Failed to determine status of hazardous waste when copy of manifest <br />signed by destination facility not received within 35 days. <br />
