A check in the amount of $2238.00 with permit fee worksheet attached. <br />2. Service Request Application as requested. <br />3. Four sets of the site plan, yard piping plan, 12,000 gallon tank and piping specification <br />("F") sheets. <br />4. A copy of a complete Bill of Materials to be supplied by Titan Rubber Company. We <br />will provide any manufacturer brochures upon request if the specifications are not <br />enough to meet the testing authority approval. <br />5. Three copies of the completed closure application (pages 3, 9, 10 and 11) forms. <br />6. The "Site Health and Safety Plan" will be provided by the contractor when selected. <br />7. We will provide the "Grading and Excavation Permit" as required by the City of Tracy <br />once their plan check is complete. <br />8.
