These Sills require <br /> the inventory, inspection and permitting of al1 underground storage tanks that <br /> contain hazardous materials. The San Joaquin Local. Health District, Division <br /> `of Environmental Health was designated as the enforcement agency for the <br /> ci.,ties and unincorporated ureas within San Joaquin County. State law provides <br /> for a fee system to cover the cost of implementing this state mandated <br /> p ray ram. <br /> Local fees •(see attached Fee Schedules) , for these required inspectional <br /> services, will. be biller.) on a yearly basis. The facilities Permit to Operate <br /> will be issued for a five year period. In addition to the yearly <br /> inspectional. fee, this statement will also include a $56/tank State surcharge <br /> fele. The State surcharge fee per, tank will be charged every five years- or <br /> whenever the facilities permit is re=newed or amended. All. State surcharge <br /> fees w:i.11 be transmitted by the Health District to the California. Wa.ter <br /> Resources Control. Board. <br /> To apply for a facilities Permit to Operate, complete the attached Fee <br /> Worksheet and submit the ,appropriates fees. Fees are due and payable 30 clays <br /> from date of this letter. <br /> In order to answer any questions regarding this Underground Storage Tanis <br /> Program, the pian Jc)aquin Local Health District, Division of Environmental <br /> Health Staff, will be available Monday-Friday between the hours of 8-1p noon <br /> and 1-5 p.m. <br /> If you have any {questions regarding this, please contact C.
