EHD was informed on February 10, 2009 that Podesta <br /> Packing as a co-owner of the same facility. Please complete the following items within 30 days <br /> of the close of escrow (Health and Safety Code section 25284(b-c). <br /> 1. Underground Storage Tanks—Facility Form—Provide all current information, sign <br /> and date at the bottom. <br /> 2. Underground Storage Tanks—Tank Forms—Provide all current information, sign <br /> and date the bottom of each form <br /> 3. Certification of Financial Responsibility/Letter from Chief Financial Officer: <br /> Complete the forms provided. This document is required to be submitted to this <br /> office annually. <br /> 4. Provide documentation(Notice of Sale or Grant Deed)indicating when escrow <br /> closed and names)of previous and new owner(s). <br /> 5. Underground Storage Tank Monitoring and Response plans: Complete and submit <br /> the provided forms. <br /> 6. Report#5021: Update all information for all programs at this site(s) including the <br /> UST and Hazardous Waste programs, sign and date. <br /> 7. Complete and return the form"Owner Statements of Designated Underground <br /> Storage Tank(UST) Operator and Understanding of and Compliance with UST <br /> Requirements." <br /> 8. Go to: http://www.waterboards.ca.gov/water issues/programs/ust to obtain the <br /> current statutes(Underground Storage of Hazardous Substances)and UST <br /> Regulations that you are required to have for your monitoring and response plans. <br /> 9.
