The fallowing <br /> are Class 11 (Repeat) Violations: 104, 204, 205 , 311, and the following are Minor Violations - Notice to <br /> Comply: 109, 201 <br /> 104 (Repeat). Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. Immediately submit a copy of the <br /> financial responsibility documents to the EHD. Alternatively, log into CERS, upload the required <br /> information, and submit for review by the EHD. Complete by 512412014. <br /> 109. EHD received the report of the April 25, 2013 monitoring system certification, leak detector testing, <br /> and spill container testing on June 28, 2013. A copy of the test report must be submitted within 30 days <br /> of the tests. Immediately begin submitting report to the EHD within 30 days of test per this section. <br /> 201. Daily alarm log was not found on site, which is needed because the alarm history print out included <br /> only alarms from April 24, 2012 and before. Jimmy also stated during this inspection there were some <br /> daily alarms logs but they were "thrown out". These records shall be kept on site for at least 3 years. <br /> Immediately locate and maintain the on site and submit copies to the EHD by 5/2412014, and begin <br /> complying per this section. <br /> 204 (Repeat). The designated operator failed to check all the alarms or if they were responded to <br /> appropriately from April 2013 to February 2014 since the attached alarm history from the Monitoring <br /> Panel recorded only up to April 24, 2012. During the monthly inspection, the designated operator shall <br /> review the alarm history for the previous month, check that each alarm was documented and responded <br /> to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report.
