The following are <br /> Class II Repeat Violations: 104, 206,303,305, and the following is a Minor-Notice to Comply: 999 <br /> 104. (Repeat) Financial responsibility documents have not been submitted to the EHD. Current financial <br /> responsibility documents are required to be submitted annually. A copy of current financial <br /> responsibility documents were provided to EHD during this inspection. Violation was corrected on site. <br /> 206. (Repeat) Documentation for the designated operator employee training for 2013 was not available <br /> or did not exist . The designated operator shall train facility employees for which he or she is <br /> responsible in the proper operation and maintenance of the UST system once every 12 months. The <br /> training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact persons) for emergencies and monitoring equipment alarms <br /> A copy of the current training documents signed today was provided to EHD by designated operator <br /> during this inspection. Designated operator stated training was performed but owner appeared to have <br /> misplace the document. Ensure that employees have been trained by the designated operator, and <br /> maintain the list and training documentation on site. Violation was corrected on site. <br /> 303. (Repeat) The date on the Monitoring Panel and the last three alarm history of all the sensors were <br /> incorrect around beginning around February 2013, possibly a result of installing a new panel and cold <br /> start during the same month, and therefore is not working properly to ensure continuous monitoring of <br /> the tank system as required by CCR 2630(d). The correct date on the Monitoring Panel was input by <br /> the technician on site. Violation was corrected on site.
