FROM: Pam Violett, San Joaquin County Environmental Health DATE: July 24 , 1991 RE: Closure Plan for Underground Storage Tanks, West Weber Stockton, CA After our telephone conversation today, I completed a review of the closure plan for the above referenced address. In addition to the items we discussed, I have found the following deficiencies: OL, 1) I am returning your check 110995 in the amount of $2 , 582 . 00. Please send a new check in the amount of $1, 582. 00 for the corrected facility, tank, state surcharge and removal fees. I have enclosed copies of the fee worksheets for your review. A one-time EPA ID number must be obtained prior to permit approval. The telephone number needed to obtain the EPA ID number is: (915) 324-1781. Please submit this number once acquired. � ) I have enclosed the Authorization to Release as it is completed +J incorrectly. The form must be signed by the OWNER/OPERATOR not an agent of your office. Also, the form must be dated. Please submit the updated Authorization to Release. 4) I have also enclosed a Tank Tracking Record. This form must be completed, to the extent possible prior to removal, and a copy returned to me prior to permit approval. bV5) Please state in writing that the amount of dry ice used to inert the tank will be 30lbs/1000 gal. 6} I have enclosed for your review this County' s sampling requirements. Please state in writing that the samples will be ;;Pbbtained 2-4 ' beneath the tank's invert. It will be your responsibility to insure the soil samples are analyzed for the applicable constituents. a\'-,7) In regard to the Health & Safety Plan submitted, you need to state who the onsite Safety officer will be, and that there will be a tailgate safety meeting prior to the beginning of the project.
