The permit <br /> application has been approved with the following conditions: <br /> 1. If the removed backfill material and soil is found to be contaminated it must be disposed of <br /> at an approved disposal location. Treatment of soil onsite can only be performed under a <br /> hazardous waste treatment permit. <br /> 2. Inert USTs with at least 30 pounds of dry ice per 1,000 gallon volume, and any additional <br /> amount if required by the Fire inspector on site. <br /> 3. A copy of the submitted"Health and Safety Plan"must be on the jobsite for review. <br /> 4. See "sampling protocol" on pages 6 & 7 of the permit application for required analytical <br /> tests and number of samples to be taken and witnessed by EHD. <br /> 5. An inspector from Stockton Fire District must be on site to verify the tanks are inert the <br /> day of the tanks' removal. <br /> 6. You are responsible for securing excavation and stock piles if not backfilled immediately. <br /> 7. Submit the following information to EHD within 30 days after the UST removal date: <br /> (a) Only analytical results that come directly from a third party California certified <br /> laboratory with affixed chain of custody will be accepted by EHD. <br /> (b) UST Tracking Record Sheet and/or Tank Hazardous Waste Manifest. <br /> (c) A copy of the completed Tank Closure Certification Form(DTSC Form 1249) <br /> (d) A copy of the Uniform Hazardous Waste Manifest of the rinsate and any hazardous <br /> waste signed by the designated facility which receives the waste. <br /> 8. See Page 2 of the Permit Application for additional general conditions. <br /> 9. This permit expires 180 days from the approval date. <br /> 10.
