A review <br /> and evaluation of the SPCC Plan must be conducted at least once every 5 years. As a result of this review and <br /> evaluation,the SPCC Plan must be amended within 6 months of review, and recertified by a Professional Engineer <br /> if any technical amendments were made. Immediately conduct a review of the facility SPCC Plan and make any <br /> necessary amendments. <br /> This is a Class II violation. <br /> 706 CFR 112.8(c)(2)Failed to provide and maintain adequate secondary containment. <br /> The two 10,000 gallon tanks were observed with insufficient secondary containment. Two small diameter holes was <br /> observed at the base of the secondary containment wall, allowing liquid from inside the containment area to drain to <br /> the outside of the containment area. All bulk storage tanks must be provided with a secondary means of <br /> containment for the entire capacity of the tank and sufficient freeboard to contain precipitation. Immediately provide <br /> sufficient secondary containment for this and all other tanks at this facility. <br /> This is a Class II violation. <br /> 710 CFR 112.8(c)(6)Plan failed to adequately discuss procedures to test or inspect each container for integrity. <br /> The SPCC plan does not specify an industry standard for the inspections and testing of the tanks.An SP-001 <br /> inspection is mentioned in the SPCC plan but only after signs of leakage are discovered. Each aboveground <br /> container shall be tested and inspected for integrity on a regular schedule and whenever repairs are made. The <br /> qualifications of personnel performing tests and inspections,frequency and type of testing and inspections that take <br /> into account container size, configuration, and design shall be determined in accordance with industry standards. <br /> Examples of these integrity tests include, but are not limited to:visual inspection, hydrostatic testing, radiographic <br /> testing, ultrasonic testing, acoustic emissions testing, or other systems of non-destructive testing. Comparison <br /> records and other records of inspections and tests must be maintained on site. Specify an industry standard that <br /> will be used for the tanks, or provide equivalence as allowed by CFR 112.7(a)(2). <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by December 6, 2018 . <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
