The guidelines spry the construction and monitor ina cr;teza <br /> necessary to complete our review of your proposed underground storage tank (UST) system. <br /> 1. SL`BMITTAL OF PLANS <br /> INSTALLATION: <br /> a. Submit 3 complete sets of drawings illustrating the locations of all existing and proposed <br /> structures, including existing UST(s), in non--erasable print to PHS-£HD. Upon approval, <br /> one set will be retained by PHS-EHD and the other two will be returned. <br /> b. Include equipment and material specification sheets and manufacrurer's brochures describing <br /> field testing procedures, operating and monitoring systems and listing numbers or other <br /> zv'dencv of a nationally recognized testing organization approval. All proposed leak <br /> detection methods and equipment must meet the requirements specified in 40 CFR 230.40 <br /> and have been reviewed by the State Water Resources Control Board Staff. <br /> c. A.11 interstitial monitors must perforin according to manufacturer's standards, and must be <br /> tested annually in accordance with section 2630 (d), California, Code of Regulations (CCR), <br /> Title 23 Waters, Division 3, Chapter 16. <br /> d. Include a detailed monitoring and response plan in accordance with section 2632 (d) (I) & <br /> (2), CCR, Title 23 Waters. Division 3, Chapter 16. <br /> e. The plans should include a cross sectional diagram of the tank and piping system including <br /> secondary containment, overflow protection equipment, and extension of all pipes and/or <br /> ports from below grade to finish grade. <br /> t. Submit a completed "PHS-EHD Service Request" form and the State Water Resources <br /> Control Board "Facility Permit (A)" and "Tank Permit (S)" application forms. <br /> SITE `MITIGATION: <br /> a. In the :vent contamination is observed, confirmed or suspected as a result of a leaking UST <br /> system it is the responsibility of the owner or operator to submit a workplan to PHS-EHD <br /> Site Mitigation Unit prior to initiating any assessment or remediation activities_ To minimize <br /> delays, a Corrective Action Plan may be submitted concurrent with tank closure or <br /> installation plans. address one copy of the Corrective action Pian to the Site i fiti;anon <br /> Unit for review.
