Financial responsibility documents have not been submitted to the EHD in a timely manner. <br /> Current financial responsibility documents are required to be submitted annually. A copy of current <br /> financial responsibility documents were submitted to the EHD at the time of inspection. CORRECTED. <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> April 4, 2013 designated operator monthly inspection report and failed to check that they were <br /> responded to appropriately. The missing alarm included: L4 alarm on March 31, 2013. During the <br /> monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history <br /> with documentation taken in response to any alarms to the monthly report. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. CORRECTED. <br /> Minor-Notice to Comply: <br /> 105 and 106. UST Facility Operating Permit Application, UST Tank Information, and UST Monitoring <br /> Plan were not uploaded in CERS. Immediately log into CERS, update the required information, and <br /> submit for review by the EHD. Complete a separate tank form for each tank. <br /> 303. The diesel sump sensor was raised approximately 4 inches up the tank saddle and off the lowest <br /> point of the sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall <br /> be maintained to be able to detect a leak at the earliest possible opportunity. At the time of inspection <br /> technician relocated the diesel sump sensor to the lowest point of the sump.
