CONDITIONS <br /> --I All samples must be taken by a representative of a state cert- <br /> ified lab. <br /> (2 ) All solids, residuals, sludges, etc. that cannot be reused as <br /> a motor vehicle fuel are to be removed, handled, transported, dis- <br /> posed of as hazardous waste. A copy of the hazardous waste manifest <br /> is to be submitted to this office within 30 days of tank removal . <br /> � 3 ) Submit receipt for final tank destination within 30 days of tank <br /> removal . <br /> 4) Obtain all necessary fire permits (with proof of permit on site <br /> at time of removal ) prior to scheduling appointment with S.J.L.H.D. <br /> 5) ' Schedule all appointments with S.J.L.H.D. at least 48 hours in <br /> advance . <br /> (6} Removal must take place within 60 days of approval date on plan <br /> F <br /> plan must be resubmitted with fees . <br /> 7 ) If water is encountered, water sample shall be obtained. <br /> Soil samples to be analyzed for : <br /> TPH-G GC-FID (5030 ) <br /> BTX&E ( 8020) <br /> EDB <br /> TEL ( IF REPORTED AS TOTAL LEAD <br /> WILL ASSUME TO BE TEL UN- <br /> LESS BACKGROUND ANALYZED) <br />
