The San Joaquinquin Local Health District, Division <br /> ted the enforcement agency for the <br /> of Environmental Health was designated as e en or <br /> cities and unincorporated areas within San Joa9uin County. State law Provides <br /> cos o <br /> for a fee system to cover <br /> thet f implementing this state mandated <br /> prngram. <br /> Local fees (see attached Fee Schedule) r for these required inspectional <br /> services, will be billed on a yearly basis" The facilities.Permit to Operate <br /> will be issued for a five year period. In addition to the yearly , <br /> inspectional fee, this statement will also include a $56/tank State surcharge <br /> fee. The State surcharge fee per tank will be charged every five years or <br /> whenever the facilities permit is renewed or amended" All State surcharge <br /> fees will be transmitted by the Health District to the California Water <br /> Resources Control Board. <br /> To apply for a facilities Permii: to operate, complete the attached Fee <br /> Worksheet and submit the appropriate ees" <br /> f Fees are due and payable February <br /> 3, 1986° Fees received after March 6' 1986 are subject to 100% penalty. <br /> In order to answer any questions regarding this Underground Storage Tank <br /> program` the San Joaquin Local Health Distict Divisibn of Environmental Health <br /> has scheduled the following information workshops: <br /> 1" Thursday, January 30r 1986' 10 AM <br /> 2° Monday, February 3, 1986, 2 PM <br /> 3" Tuesday' February 11' 1986, 2 PM <br /> All of the above informational workshops will be conducted at the San Joaquin <br /> Local Health District Board Room/ 1601 E. Hazelton Ave. , Stockton, CA.
