Box 2009 <br /> Stockton, CA 9.5201 <br /> (209) 466-6781 <br /> Jogai rhanna., M.D., Health Officer <br /> January 13, 1986 <br /> JAMAR40 <br /> JAMAR SERVICE JAMAR SERVICE <br /> <br /> STOCKTON, CA 95205 <br /> In late 1983, the Governor signed into law AB2013 and 1362. These Bills require <br /> the inventory, inspection and permitting of all underground storage tanks that <br /> contain hazardous materials. The San Joaquin Local Health District, Division <br /> of Environmental Health was designated as the enforcement agency for the <br /> cities and unincorporated areas within San .Joaquin County. State law provides <br /> for a 'fee system to cover the cost of implementing this state mandated <br /> program. <br /> Local fees (see attached Fee Schedule) , for these required inspectional <br /> services, will be billed on a yearly basis. The facilities Permit to Operate <br /> will be issued for a five year period. In addition to the yearly <br /> inspectional. fee, this statement will also include a X56/tank State surcharge <br /> fee. The State surcharge fee per tank will be charged every five years or <br /> whenever the facilities permit is renewed or, amended. All State surcharge <br /> fees will be transmitted by the Health District to the California Water <br /> Resources Control Board. <br /> To apply for a facilities Permit to Operate, complete the attached Fee <br /> Worksheet, and submit the appropriate fees. Fees are due and payable February <br /> 3, 1986. Fees received after March 6, 1986 are subject to 100% penalty. <br /> In order to answer any questions regarding this Underground Storage Tank <br /> program, the San Joaquin Local Health. Distict Division of Environmental Health <br /> has scheduled the following information workshops: <br /> 1. Thursday, January 30, 1986, 10 AM <br /> 2. Monday, February 3, 1986, 2 PM <br /> 3. Tuesday, February 11, 1986, 2 PM ' <br /> All of the above informational workshops will be conducted at the Sari Joaquin <br /> Local Health District Board Room, 1601 E. Hazelton Ave. , Stockton, CA. Due to <br /> limited space, these workshops wil. be limited to the first 200 persons per <br /> meeting. <br /> If you have any questions regarding this, please contact C.
