Hwy. 99 Frontage Rd., Stockton <br />The permit application to remove a 10, 000 gallon diesel underground storage tank (UST) <br />039-0515837, associated piping and other appurtenances from the above referenced <br />facility has been approved with the following conditions: <br />1. If the backfill material and soil is found to be contaminated it must be disposed of at <br />an approved disposal location. Treatment of soil onsite can only be performed under a <br />hazardous waste treatment permit. <br />2. A copy of the "Site Health & Safety Plan" must be on the jobsite for review. <br />3. See pages 7 & 8 of the permit application for required analytical tests and number of <br />samples to be taken and witnessed by this department. Form EH 23 046 Revised <br />08/03/07 must be utilized. <br />4. A copy of the fire department permit must be submitted at the time of removal. <br />5. In addition, the following information must be submitted within 30 days after the <br />UST removal date: <br />(a) Only analytical results that come directly from a third party California certified <br />laboratory with affixed chain of custody will be accepted by this office. <br />(b) UST Tracking Record Sheet and/or Tank Hazardous Waste Manifest. <br />(c) Tank Closure Certification Form (DTSC form 1249). <br />(d) Hazardous Waste Manifest for piping, rinsate, residual fuel, or waste oil receipt. <br />6. This permit expires on November 20, 2007. <br />7.
