PUBLIC HEALTH SERVICES <br /> SAN JOAQUIN COUNTY Z: , <br /> ENVIRONMENTAL HEALTH DIVISION V <br /> Karen Furst, M.D., M.P.H., Health Officer <br /> q(iFopN <br /> 304 East Weber Avenue, Third Floor • Stockton, CA 95202 <br /> 209/468-3420 <br /> TO: INSTALLERS AND OWNERS OF NEW UNDERGROUND STORAGE TANKS <br /> San Joaquin County Public Health Services-Environmental Health Division(PHS-EHD)has established the following guidelines to <br /> assist you in the completion of your construction project. The guidelines specify the construction and monitoring criteria necessary <br /> to complete our review of your proposed underground storage tank(UST) system. <br /> 1. SUBMITTAL OF PLANS <br /> INSTALLATION: <br /> a. Submit 3 complete sets of drawings illustrating the locations of all existing and proposed <br /> structures, including existing UST(s), in non-erasable print to PHS-EHD. Upon approval, one <br /> set will be retained by PHS-EHD and the other two will be returned. <br /> b. Include equipment and material specification sheets and manufacturer's brochures describing <br /> field testing procedures, operating and monitoring systems and listing numbers or other <br /> evidence of a nationally recognized testing organization approval. All proposed leak detection <br /> methods and equipment must meet the requirements specified in 40 CFR 280.40 and have been <br /> reviewed by the State Water Resources Control Board Staff. <br /> C. All interstitial monitors must perform according to manufacturer's standards, and must be <br /> tested annually in accordance with section 2630 (d), California Code of Regulations (CCR), <br /> Title 23 Waters, Division 3, Chapter 16. <br /> d. Include a detailed monitoring and response plan in accordance with section 2632 (d) (1) & (2), <br /> CCR, Title 23 Waters, Division 3, Chapter 16. <br /> e. The plans should include a cross sectional diagram of the tank and piping system including <br /> secondary containment, overflow protection equipment, and extension of all pipes and/or ports <br /> from below grade to finish grade. <br /> f. Submit a completed "PHS-EHD Service Request" form and the State Water Resources Control <br /> Board "Facility Permit (A)" and "Tank Permit (B)" application forms. <br /> SITE MITIGATION: <br /> a. In the event contamination is observed, confirmed or suspected as a result of a leaking UST <br /> system it is the responsibility of the owner or operator to submit a workplan to PHS-EHD Site <br /> Mitigation Unit prior to initiating any assessment or remediation activities. To minimize <br /> delays, a Corrective Action Plan may be submitted concurrent with tank closure or installation <br /> plans.
