The permit application has been approved <br /> with the following conditions: <br /> ' 1. If the removed backfill material and soil is found to be contaminated it must be disposed <br /> of at an approved disposal location. Treatment of soil onsite can only be performed under <br /> a hazardous waste treatment permit. <br /> t2. Inert USTs with at least 30 pounds of dry ice per 1,000 gallon volume per Stockton Fire <br /> Department, and any additional amount if required by the Fire inspector on site. <br /> 3. A copy of the submitted"Health and Safety Plan" must be on the jobsite for review. <br /> ' 4. See"sampling protocol" on pages 7 & 8 of the permit application for required analytical <br /> tests and number of samples to be taken and witnessed by EHD. At least 4 samples are <br /> required. <br /> ' 5. A copy of the Fire Department permit or an official from the Fire Department must be <br /> available on site at the time of removal. <br /> 6. You are responsible for securing excavation if it will not be backfilled immediately. <br /> 7. Submit the following information to EHD within 30 days after the UST removal date: <br /> (a) Only analytical results that come directly from a third party California certified <br /> ' laboratory with affixed chain of custody will be accepted by EHD. <br /> (b) UST Tracking Record Sheet and/or Tank Hazardous Waste Manifest. <br /> (c) A copy of the completed Tank Closure Certification Form (DTSC Form 1249) <br /> ' (d) A copy of the Uniform Hazardous Waste Manifest of the rinsate and any hazardous <br /> waste signed by the designated facility which receives the waste. <br /> 8. This permit expires 180 days from the approval date. <br /> ' 9.
