Page 1 of
Jeff Wong [EH]
From: Jeff Wong [EH]
Sent: Tuesday, October 18, 2011 2:27 PM
To: 'frank.poss@ia psiusa.com'
Subject: Tank removal forms on SJCEHD website
Hi Frank:
This is the section on our Department's website where the forms for UST removal can be found:
http-/Iwww.sjgov.org/ehd/Programs/Unitlll./`.u-.nj-t.iii.htm
Including to the permit application for tank removal 1 close in place, please be sure to include A and B forms so our office
can generate a TA#for the tank. I still have to talk to my supervisor(who's not in at the moment) regarding backfees. All
said and done, the tank removal 1 close in place is only one part of the puzzle, as where you're trying to get to in the end is
a "No Further Action" letter from us, unless there's evidence(soil sample analyses, site observation, etc.)that warrants
additional investigation.
Best Regards,
Jeffrey C.F. Wong, Sr. REHS
Senior Registered Environmental Health Specialist
10/18/2011
