The permit has been approved with the following conditions: <br /> 1. Contact the local fire department, and the San Joaquin Valley Air Pollution Control District for <br /> applicable permits and requirements. <br /> 2. Any changes in information or scope of work must be submitted to EHD prior to commencing <br /> work. <br /> 3. All piping and connections shall be approved by an independent testing organization and <br /> bear an approval label or marking(example: UL label). <br /> 4. At least one person who holds the International Code Council"UST Installer/Retrofitter" <br /> certification must be onsite AT ALL TIMES when work is performed. <br /> 5. Contact Steven C. Shih at(209)468-9850 to arrange the following inspection appointments; a 48- <br /> hour notice for inspection appointments is required. <br /> a. Inspection for the existing piping condition before the existing piping is touched or removed <br /> from piping trench. <br /> b. Samples. The following information must be submitted within 30 days of the sample date: <br /> L Analytical results mailed directly from a California certified laboratory or this office will <br /> not accept them. <br /> ii. See attached page 4& 5 of the permit application for required analytical tests. <br /> c. Piping test: pressure and soap test on primary piping, secondary piping; and piping slope. <br /> d. Lake Test: 24-hour lake test on UDCs and turbine sumps. <br /> e. Corrosion protection: prior to backfilling around the spill buckets, corrosion protection of <br /> the risers must be verified. <br /> f. Overfill prevention: overfill prevention system in compliance with CCR 2635 (b)(2)must be <br /> verified. <br /> g. Spill Bucket: after concrete is poured, spill bucket with minimum of 5 gallon capacity and a <br /> 30-minute lake test in compliance with CCR2635(b)(1)must be verified. <br /> h. Third party tank integrity test and piping line tightness test must be performed once <br /> concrete is poured,prior to the contractor signing off of the jobsite. The test results must be <br /> submitted to EHD within 7 days of the test. <br /> i. Final inspection: final inspection shall be performed to insure all components of <br /> underground storage system are functional before this facility is back to operation.
