Main St., Stockton, CA 95205 <br /> The San Joaquin County Environmental Health Department has been informed that you are the new owner or <br /> operator of the underground storage tanks (UST's) at the above-mentioned facility. Please complete the <br /> following items within 30 days of the close of escrow[CA Health and Safety Code section 25284 (b) & (c)]: <br /> 1. Create an account and obtain facility access to the facility on the California Environmental Reporting <br /> System (CERS) by contacting the lead user on the account and going to hftp://cers.calepa.ca._qov. <br /> Ensure that all required uploads are completed in a timely manner. <br /> 2. Underground Storage Tanks — Facility Form — Provide all current information. (Note: Any change of <br /> information must be updated in CERS within 30 days of the changes. Log into CERS, update the <br /> required information, and submit for review by the EHD. Complete a separate tank form for each tank. <br /> 2. Underground Storage Tanks—Tank Forms— Provide all current information in CERS. <br /> 3. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete the forms <br /> provided, and upload into CERS. This document is required to be uploaded into CERS annually. <br /> 4. Underground Storage Tank Monitoring and Response Plans: upload in CERS. Retain an approved <br /> copy of these plans onsite for future reference. <br /> 5. Report #5021: Update all information for all programs at this site including the UST and Hazardous <br /> Waste programs, sign and date. Bring with all other forms to our accounting office to pay for transfer <br /> fees. If you want a one hour consultation with the EHD, there is a UST Consulting fee of$130/hr. <br /> 6. If you are the owner, you must submit a notification form identifying the "Designated UST Operator" for <br /> each facility owned. A notification form is enclosed for your use. Upload this document into CERS. <br /> 7. Go to: http://www.waterboards.ca.gov/cwphome/ust/regulatory/index.html - rqs to obtain the current <br /> statutes (Underground Storage of Hazardous Substances) and UST Regulations that you are required <br /> to have for your monitoring and response plan. <br /> 8. Also enclosed for your information is a brochure of Best Management Practices (BMP's) for Retail <br /> Gasoline Outlets that you can use as part of your BMP's for your facility. <br /> 9. Hazardous Materials Management Plan: Contact EHD Hazardous Material Staff at (209) 468-3420 for <br /> information on updating this plan. <br /> 10. Information and instructions for obtaining a permanent California EPA ID number (enclosed hereto).- <br /> Complete <br /> ereto):Complete the application for a California Hazardous Waste Permanent ID Number and fax it to (916) <br /> 255-4703. <br /> 11. Board of Equalization UST Storage Fee Account Number — Call (800) 400-7115 to obtain a current <br /> number for your facility. <br /> 12. San Joaquin Valley Air Pollution Control District— Call (209) 557-6400 to obtain an operating permit for <br /> the UST vapor recovery system <br /> 13. Food facility operations — Call EHD Food Staff at (209) 468-3420 for operating permit for opening the <br /> retail food facility. A consultation appointment may be needed. <br /> 14.
