The monitoring plan is not current and/or not approved by the EHD. The 91 monitoring plan lists the pipe monitoring panel model as RJ 116-035, but should be listed as EMC. The 91 leak detector model was listed as FX1 V, but should be XLP. All tanks list the pipe monitoring leak sensor model as 208, but should be 205. The monitoring plan must be uploaded to the California Environmental Reporting System(CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class II. 114 CCR 2715(a) Failed to inform the EHD of designated operator identification or change within 30 days. A new designated operator, Shawn Rodriguez was hired on 5-25-16 and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS)at hftp://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a minor violation. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years are incomplete. The facility and designated operator did not document the 11-13-15 L5 87 annular fuel alarm. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection.
