If the backfill material and/or soil is found to be contaminated it must be disposed of <br /> at an approved disposal location. Treatment of soil onsite can only be performed <br /> under a hazardous waste treatment permit. <br /> 2. See pages 7 & 8 of the permit application for required sampling and analytical tests. <br /> 3. A copy of the fire department permit must be submitted at the time of removal. <br /> 4. Schedule removal appointment 48 hours in advance. <br /> 5. Sampling firm shall be an independent third party from the contractor. <br /> 6. In addition, the following information must be submitted within 30 days of the <br /> removal date: <br /> (a) Analytical results, mailed from the lab <br /> (b) UST disposition tracking record and tank hazardous waste manifest <br /> (c) Tank closure certification form <br /> (d) Hazardous waste manifest for piping and rinsate <br /> 7.
