Form A, permit application dated 7/1/91 . <br /> 2 . Form B, permit applications ( 3) 1 per tank, dated 7/1/91 . <br /> 3 . Installation drawings: 4 sets, also includes site plan <br /> and vapor recovery drawings. <br /> 4 . Proof of financial responsibility (coverage period <br /> through 5/1/92) . <br /> 5. Written fill procedures (SOHIO, 1982, still in effect) . <br /> 6. Monitoring/response plan (dated 1/15/91) . <br /> 7. Emergency response plan dated June 1 ,1990. <br /> B . Letter of commitment date 1/15/91. <br /> 9. Cut sheets for all installed equipment, piping, sensors, <br /> and tank etc. . . <br /> 10 .
