The January 2014 monthly <br /> Designated Operator 0.0)report stated that a service company filled the 87 fill brine and the diesel fill brine.There <br /> were no maintenance records addressing the filling of the brine as referenced in the January 2014 monthly D.O report <br /> These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all <br /> monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3)results of any visual <br /> observations, (4) results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other <br /> monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br /> reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site <br /> and submit copies to the EHD. This is a repeat violation, Class II. <br /> 206 HSC 25294, 25295 Failed to record or report unauthorized release in appropriate time frame. <br /> The 11/12 dispenser had an active leak from the primary piping that escaped the secondary containment. Alarge <br /> water product mixture stain on top of the concrete was noticed, and some dripping from the diesel nozzle. The <br /> technician on site used absorbent pads to clean the leak that spilled on the outside of the UDC and placed the <br /> generated waste inside of hazardous waste container. Any release from the primary containment that does not escape <br /> the secondary containment and was not cleaned up within eight hours of detection, or any release that escapes the <br /> secondary containment shall be reported to the EHD within 24 hours and a full report shall be submitted within five <br /> working days. The report shall include at least the following information: <br /> 1. Owner's or operator's name and telephone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Approximate date of release <br /> 4. Date release was discovered <br /> 5. Date release was stopped <br /> 6. Description of actions taken to control and stop the release <br /> 7. Description of corrective and remedial actions <br /> 8. Methods of clean up implemented to date, and proposed clean up actions with a time schedule <br /> 9. Method and location of disposal of waste generated from release and clean up. Attach copies of any hazardous <br /> waste manifests. <br /> 10. Description of proposed methods of repair or replacement of primary or secondary containment <br /> 11. Description of additional actions taken to prevent future releases. <br /> Immediately prepare a report with the required information and submit a copy to the EHD. Ensure that any future <br /> releases are reported appropriately. This is a repeat violation, Class II. <br /> 305 HSC 25290.1(e)VPH monitoring of the interstitial spaces of the UST system is not maintained. <br /> The interstitial brine sensor(1-2)in UDC 1/2 failed when tested. The sensors monitoring the liquid level in the <br /> interstitial space shall be maintained so that a breach in the primary or secondary containment is detected before the <br /> hazardous substance is released into the environment. The sensor was replaced and retested during the inspection. A <br /> "UST Retrofit Verification with Inspector Already Onsite"form has been completed and provided to the operator and <br /> contractor.According to the technician the (1-4)brine sensor in UDC 3/4 failed,the technician disconnected the wires in <br /> the junction box and reconnected them retested the sensor with passing results.
